Description
Statement of Work:

Contractor to provide to Iron Mountain VAMC, 325 East H Street, Iron Mountain, MI 49801-4760 Catholic Services as stated in the Schedule of Supplies/Services. Chaplain duties will include but not be limited to the following:

A. Conduct Eucharistic Celebrations to include weekly Mass, Communion, to bedside as requested, complete Eucharistic Celebration report, 5 Holy Day of Obligation Masses, Ash Wednesday Service, and Good Friday Service (maximum of 58).

B. Respond to patient and staff requests for assistance promptly & in a friendly & cooperative manner.

C. Listen to feedback from patients/staff, positive and negative, acts to resolve complaints within his control, and reports feedback to COR in a timely manner.

D. Provide verbal information that is thorough and correct, using clear and precise communication methods to ensure that others understand the. The any required information is provided in a timely manner.

E. Consistently has open dialogue with other chaplains as it relates to Chaplain Service.

F. Cooperates with other Chaplains/Deacons to ensure that veterans receive proper care from the Chaplain Service Volunteers.

G. Catholic Priest interacts with the Hospital s multidisciplinary health care team and are expected to share relevant, non-confidential information with other disciplines. Non- confidential issues that pertain to the Veteran s spiritual health care planning must be shared with fellow providers in-order to ensure continuity of care.

H. All Chaplain Service Volunteer contacts will be included in workload report and given to the Voluntary Service office.

I. Provides religious services held in the Iron Mountain VA Chapel.

J. Arranges for the atmosphere and symbolism of worship. Provides religious instruction, such as individual or group discussions, instructional classes, and distribution of appropriate literature to patients as required.

K. Room 1182, Chapel Office is provided for the Catholic Priest to use in performance of duties as outlined in the statement of work.

L. Room 1284, Chapel is provided for performance of weekly Mass services.

The Contracted Catholic Priest will in advance, prepare a monthly routine schedule for services provided under this contract, based on the number of visits the Contractor will conduct per month, with services generally being provided on the same days each week of the month. Report will be presented to COR for approval prior to beginning of the month.

SPECIAL CONTRACT REQUIREMENTS
a. The services specified herein may be changed by written modification to this contract. The VA Contracting Officer will prepare the bilateral modification (ref. FAR clause 52.212-4(c), Changes.
b. There is no employer-employee relationship between the VA and the contractor or the contractor s employee(s).
c. Must be an Ordained Catholic Priest who possesses an ecclesiastical endorsement, from the official national endorsing authority of their faith group or denomination. The ecclesiastical endorsement is a written official statement, by the official national endorsing body of the faith group or denomination, certifying that the applicant is in good standing with the applicant s group or denomination; and stating that the individual is, in the opinion of the endorsing body, qualified to perform the full range of ministry required in the VA pluralistic setting.
d. The Ordained Catholic Priest shall have the training of their religious faith group that is in keeping with their established standards.
e. Personnel assigned by the contractor to perform the services covered by this contract shall be proficient in written and spoken English (38 USC 7402).
f. Contractor s assigned personnel must be computer literate as patient documentation is directly entered in the patient s electronic medical record.
