Solicitation Number / RFQ 36C24422Q0620 Fitness and Wellness Services for Veterans at the James E. Van Zandt VA Medical Center (VAMC) in Altoona, Pennsylvania.
This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, using Simplified Acquisition Procedures found at FAR Subpart 13.5 and as supplemented with additional information included in this notice.
This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.
The combined synopsis solicitation number is 36C24422Q0620. It is issued as a Request for Quote (RFQ). The government anticipates awarding one five-year firm-fixed price Blanket Purchase Agreement (BPA) resulting from this solicitation. The James E. Van Zandt VA Medical Center (VAMC) is seeking a five-year firm-fixed price Blanket Purchase Agreement (BPA) for Fitness and Wellness Services for eligible Veteran patients to support and supplement existing programs. The Contractor agrees to furnish one hundred fifty (150) individual adult, annual memberships to be used at their facility for eligible Veteran patients receiving services through the James E. Van Zandt VAMC. The Contractor must be capable of meeting all requirements in the Statement of Work (SOW) attached to this Solicitation.
The James E. Van Zandt VAMC intends to award a contract for five years, with year one to begin on or about 02/01/2023 01/31/2024.
