Statement of Work



Dis-Assembly and Removal of Four Lektrievers






  1. GENERAL REQUIREMENTS: Contractor to disassemble Lektrievers and remove them from the property to be sent to a certified Recycling Facility.






  1. Site Address: U.S. Coast Guard Finance Center, 1430 A. Kristina Way, Chesapeake, VA 23326.

  2. Facility Hours: 0800-1500





2. CONTRACTOR REQUIREMENTS: Requesting GSA pricing from a GSA scheduled contractor. Contractor must be able to dis-assemble four Lektrivers; model numbers: LK-11611-14, LK-10211-14, LK-13011-14, and LK-07711-14, for which parts will be sent to a certified recycling facility, with a letter of destruction from this facility being sent to Vincent.C.Burke2@uscg.mil.





3. SCOPE OF WORK:

To dis-assemble and remove four Lektrievers (electronic filing cabinets) from the USCG Finance Center, and the parts to be sent to a certified recycling facility. Certified Recycling Facility means a recycling, composting, materials recovery or re-use facility determined to process incoming C&D materials to divert from landfill or transformation for which the certifying agency has issued a certification. The four Lektrievers are located right next to a single door entrance that is 33 inches in width and 81 inches in height. There is a double door entrance about 100 feet way that is 70 inches in width and 81 inches in height. For parts to be removed from the building, the four Lektrievers will need to be dis-assembled. The Lektrievers are Kardex models, with model numbers of: LK-11611-14, LK-10211-14, LK-13011-14, and LK-07711-14. I am requesting firm fixed pricing under GSA Contract, and pricing should be all inclusive.





4. DESIGNATED POINT OF CONTACT: The designated point of contact is SK1 Vincent Burke. 757-523-6710. Vincent.C.Burke2@uscg.mil.





5. CG FACILITY SITE: The US Coast Guard Finance Center (FINCEN) has parking at no cost. There is no gate access or rapid gate pass required. All contractors will check in with the security at the main entrance of the Finance Center to receive a visitors’ badge. All contractors will receive an escort throughout the building. Building has badge access to enter inside of the facility. No security clearance is required. There is one single door right next to the four Lektrievers that is 33 inches wide with a height of 81 inches. There is a double door entrance to the right of the four Lektrievers about 100 feet way. The double door specifications are 70 inches in width and 81 inches in height. Lektrievers must be dis-assembled for parts to fit through the single or double doors.



6. CONDITIONS AFFECTING THE WORK: The Offeror should visit the site and take such other steps as may be reasonably necessary to ascertain the nature and location of the work, the general and local conditions which can affect the cost of the work thereof. Failure to do so will not relieve bidders from responsibility for estimating properly the difficulty or cost of successfully performing the work. The government will assume no responsibility for any understanding or misrepresentations concerning conditions made by any of its officers or agents prior to the execution of the contract, unless included in the request for quotes, the specification or related documents.





7. ORAL MODIFICATION: No oral statement of any person other than the contracting officer shall in any manner or degree, modify or otherwise affect the terms of this contract.





8. WORKMANSHIP: All work shall be accomplished with the best standard practices, and by workmen thoroughly experienced in the required trades. The work shall be supervised by experienced personnel and shall be satisfactory to the Contracting Officer.





9. PERMITS: The contractor shall without additional expense to the government, obtain all appointments, licenses, and permits required for the execution of the work. The contractor shall comply with all applicable federal, state and local laws.





10. WORK OUTSIDE REGULAR HOURS: If the contractor desires to carry on work on Saturday, Sunday, holidays or outside the unit’s regular hours, must submit their request in writing to the Contracting Officer’s QAE Representative for approval consideration. The contractor shall allow ample time to enable satisfactory arrangements to be made by the government for inspecting the work in progress.





11. UPON COMPLETION OF WORK: Contact the Contracting Officer’s Representative to arrange for inspection/acceptance of work.





12. IDENTIFICATION OF CONTRACTOR PERSONNEL: The contractor shall provide a detailed list of all employees to SK1 Vincent Burke, five (5) days prior to commencing work. The employee list shall contain the employee’s full name. The onsite Security Officer will be given a copy of the employee list and a pass will be issued for entry.
