This notice is being issued in an attempt to determine the level of interest among all socioeconomic groups. Responses will assist VA planners in determining an appropriate acquisition and set-aside strategy which best fits VA requirements.

The Wilkes-Barre VA Medical Center (VAMC) located at 1111 East End Blvd, Wilkes-Barre, PA 18711 is performing market research to determine if there is a sufficient number of qualified: (1) Service-Disabled Veteran Owned Small Business (SDVOSB); (2) Veteran Owned Small Business (VOSB); (3) Small or emerging small business firms (SB or other); or (4) large business (LB) firms to set aside a single award, five year Agreement for Whole Health, Arts and Meditation Group Facilitator. If applicable, SDVOSB and VOSB must be verified in SBA.

The Wilkes-Barre VAMC is seeking a five-year, firm fixed Blanket Purchase Agreement (BPA) to offer Complementary and Integrative Health (CIH) options to the patient as part of his or her treatment plan. Attached is the Draft Statement of Work (SOW) in accordance with this requirement.

The anticipated period of performance is to start April 17,2023, for a total of five years.

The North American Industry Classification System (NAICS) code being considered for this procurement is 621340.

All interested firms who can meet the requirement as outlined in the Draft SOW, should respond in writing, and provide the following information:
1. General
a. Explain your Socioeconomic status.

2. Describe your firm s capability to perform the tasks as outlined in the SOW and include:
a. Company name and address
b. List the days/hours of operation for your facility/program.
c. If registered in System for Award Management (SAM), provide Unique Entity Identifier (UEI).
d. List Point of Contact information (i.e. title, phone number and email address)
e. Are you planning on subcontracting, if so what part(s) of the SOW?
f. Describe any additional relevant information.
g. Please provide any addition NAICS codes that you deem appropriate.

3. Existing Contracting Vehicles: Do you have a GSA FSS contract or other applicable contract vehicle if within scope of this effort (if applicable)? If so, indicate contract number, SIN, expiration date, etc. and include how your existing contract is within scope of this requirement.

4. Are you currently enrolled SAM: https://sam.gov/content/home
Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications required in the Systems for Award Management (SAM) database prior to award of a contract.

All responses, including capability statement, shall be submitted in writing via email no later than 1:00 PM (ET), Tuesday, March 7, 2023, to Ms. Arlene Albrite, Contract Specialist (CS).
